I have an equation in Chapter 1 that I would like to repeat in Chapter

Changing the label in the figure to SAMPLE-TABLE will solve your problem.
Another solution, is to make the heading appear in normal case instead
of uppercase. This
will probably be
selected, un-select this and then choose the one that you want (Here
Definitely).

This should put force them to appear in that order, starting from the
top, and anything not explicity listed will follow. That way, you can
put the most frequently used styles near the top (or bottom) of the
list, where they are easier to find.

I don't know how to do a centreline symbol (a C overlapped by an L).
Anyone know of a LaTeX code / package for this?
