This functionality only works during the AppleTV setup phase. During setup, it will say, Triple-click Play button to activate VoiceOver. Once the AppleTV is setup, this functionality is no longer available. That was straight from www.lioncourt.com, but it doesn’t say where he got that

I need to apologize to you. I just tried it on my Apple TV and low and behold, the triple click does not work on my unit anymore. I don't know when this option was changed but the last time I had to turn Voice Over on, it was working just fine. I sure hope that they will give this option

On the subject of turning Voice Over on and off, is there a way to have Voice Over start automatically when my Mac is turned on? I know that you can have it set to speak at the log in screen, but I have my Mac set to go right to the desktop and in order to get speech I need to hit command F5.

This is a limitation of the Windows DRM itself. Microsoft has not released code or libraries to their DRM for the Mac so nobody can make a player that will open a DRMed Windows Media file. Not a problem the Flip4Mac folks can fix since the critical bits of info are locked up by Redmond.
