If you need to be able to use html, you could just add some logic to the
templates with the address information. Of course the number of locations that
you have may render this solution impractical.

As to the tear off block, a couple of years ago we set up the checkout receipt
to print a raffle ticket at the bottom of the receipt.
We used the following with the raffle ticket information, I believe it
was patron id below this
Then we set up the printer driver to cut at a page break.
I need to figure out how to take out some of the space above the image, but
it's a start.
