We've managed to figure out that this is related to sendmail .forward files on the remote servers - there's something about the way sendmail does the headers when using .forward that qmail / ezmlm doesn't like. We've worked around it by sending LogWatch reports directly to our mail list

Hello! I just want to point out that there might be a bug: In the context of maildir quotas, qmailadmin does not count subdirectories of the users Maildir. If using IMAP folders it might matter. Since I'm not using quotas, I did not investigate this furter. Many regards, claas
