Subject: [Sepsis Groups] Sepsis Coordinator
Importance: High

Looking for experience and best practices from hospitals that have implemented
a sepsis coordinator position. Specifically, please reply to the following
questions:

*Hospital stats - how many sepsis patients do you see monthly? How many
beds do you have at your hospital? How long have you had a sepsis coordinator
position in place?

*What exactly does the sepsis coordinator do and what are their
specific responsibilities?

*How much time each month do they spend on sepsis coordination?

*Do they have additional responsibilities or is their role 100% sepsis
only?

*Have you experienced any improvement in data metrics after
implementing a sepsis coordinator position, such as SEP-1 compliance, sepsis
mortality, sepsis LOS, sepsis readmissions, etc.?

Any information that anyone can provide would be very much appreciated!

Thanks!

Katie,
I am the Sepsis Coordinator at Navicent Health. We are a 630 bed facility,
Level 1 Trauma Center, Level 3 NNICU, we have 5 adult ICUs and a Pediatric ICU.
I have been in this position since April and August 1st it became my full time
job. There are a lot of moving parts and multiple changes that need to
continuously be made. The position requires a lot of administrative backing to
succeed, which luckily I do have.
We also have a Sepsis Team that meets every other week to discuss all issues
related to the care of septic patients. The team consists of over 40 members
consisting of: managers, directors, administration, quality, lab, pharmacy,
nursing leaders, respiratory, coding, utilization review, MD champions,
discharge planners, NPs, information services analysts, EC, ICU, and Med surge
leaders as well as bedside nurses.
I will confirm it is definitely at least enough work for one person no matter
what size the facility is.
Good luck! ☺

Hi,

I have a split position; 50% CNS critical care and 50% sepsis coordinator at

I have support with data analyst but am still hand collecting some data points
that we can’t seem to pull automatically.

My JD is a bit muddled since I have 2 roles. I pulled out some of the sepsis
statement.

nurses, and other health care professionals for various patho-physiologic
statuses common to the critical care and sepsis patient.

Utilizes teaching/learning principles coupled with an expert clinical knowledge
to promote and provide education to the staff, patient, and patient's family.

Acts as a consultant to other disciplines in the area of critical care and/or
sepsis by providing a high level of current, research-based, clinical expertise
and advanced decision making skills.
Collaborates with medical director of the sepsis team regarding administrative
issues that require intervention and/or resolution.

Provides formal and informal critical care and sepsis-related inservice and
educational programs in which expert knowledge base is shared with staff.

Promotes growth toward higher level of professionalism within Christiana Care
Health Services continuum of sepsis and critical care through role modeling and
expert practice.

Coordinates select projects/programs, including development and implementation
of clinical pathways, guidelines and protocols, in collaboration with the team
leader of the sepsis program and director of critical care.

Ensures compliance with regulatory standards for critical care and sepsis

Works with performance improvement department on continuous improvement for
critical care and sepsis patients within CCHS.

Maintains collaborative professional working relationships with clinicians and
administrative staff necessary to develop and implement a successful cross
continuum critical care and/or sepsis program.
Demonstrates skills and knowledge necessary to provide care to sepsis and
critical care patients including knowledge of growth and development, the
ability to obtain and interpret information, to identify patient needs and to
provide care needed.

Katie,
I am the Sepsis Coordinator at Navicent Health.
Good luck! ☺

Re: [Sepsis Groups] Sepsis Coordinator

2016-07-19 Thread Medina, Rebeka - SJPVH - 001
I worked as the 'unofficial' Sepsis coordinator at both campuses of our
hospital for 2 + years. I was full time, but had other duties assigned as
well. In that time we drastically reduced our sepsis LOS as well as our
mortality rates. There was requests made by our physician groups to make this
a full time position, but it was not in the budget. However, I firmly believe
that there could have been even more of an impact made if this had gone
through. There was just never enough time to educate all of the nursing staff.

Good Luck!

Re: [Sepsis Groups] Sepsis Coordinator

2016-05-14 Thread Wightman, Mary
Good Day-

We are currently looking at resource possibilities for a sepsis coordinator
position(s).
Does your institution have such a position? Is it a dedicated resource- not
considered "other duties as assigned"? Also, is it a full or part time role?

Many thanks in advance for your response-
Mary

Re: [Sepsis Groups] sepsis coordinator roles

2015-09-17 Thread Barnes-Daly, Mary Ann
We at Sutter Health have transitioned to the Quality dept for several reasons;

1. These individuals are very focused and experienced in the area of CM
abstraction. There is a lot of carryover from other measures that they already
know; and they are very objective on the criteria (once CMS releases yet
another revision in the next few weeks).

2. Because the denominator now contains ALL sepsis cases; simple sepsis,
severe sepsis and septic shock, many of the cases are not ever in the realm of
cases that we want to review - so the sepsis coordinators can spend time on
actual severe sepsis and septic shock cases - and not doing data abstraction
for simple sepsis cases
This said, the sepsis SMEs and clinical folks are very engaged in sharing
information and collaborating to be sure that the abstractors and coders have
all of the new, pertinent and important information required.

Thanks,
