Recently I have had 2 oVirt hosts (oVirt 4.2, CentOS 7.5) crash
unexpectedly (not at the same time). Both seem hardware related.
In both cases oVirt did detect the host as non responsive, did a
fence on the hosts and set the VMs which were running on the host at the
time as "Down".
I actually do not agree with Simone here. The fix he talks about adds
a call to prepareImage, but your log clearly shows that prepareImage
is the call that fails:

i've ovirt nodes connected to a data network (provide disk for guest) with 10Gb
networking.
Recently i face a problem : one of my node have lost connectivity to the SAN.
So ovirt put all guest in "Pause" state, but when connectivity came back ovirt
did not restart the Guest.

use KVM solution in our environment)
Have 2 hosts with NFS storage and i tested migration between these two
hosts.
On Host2 while VM was running on it, i unplugged Power cable and it took
long minutes to update that VM
