You don't need SIRI to voice dial 911. you can just press and hold the home
button until the phone vibrates and dings, then say dial (or call) 9 1 1.
That's it. Voiceover will say dialing 9 1 1 (assuming it heard you correctly)
and proceed to make the call.

This has happened to me several times since owning iPhones (3GS, and now a 4).
The problem has always been lint, or some other obstruction in the microswitch
of the earphone jack. When you insert the earplugs into the jack, it closes, or
opens, (I can't remember which) a small switch that
do a search for the app from your search screen (left of the first home
screen). That'll tell you if its actually on the iPhone, If you've got the name
right that is. This happened to me once and it turned out that the app wound
up in a subfolder somehow and was the reason I couldn't find it
