I had this happen to me today when trying to place a call. My phone freaked 
totally out and no matter what I tried, I could not get Voice Over to work. I 
had to reboot the phone and even then I had trouble getting Voice Over back. 
Somehow or another after a little wait, it fixed itself. I sure hope this gets 
fixed soon. The call I was trying to make had to be made at a certain time and 
I was just unable to make that call for about 5 minutes. I know bugs are a 
norm, but it just seems like they rush out these updates just because a new 
version of the iPhone is released. I knew I should not have updated so quickly, 
but I was eager to get the Silence Unknown Callers feature that comes with IOS 
13. This has definitly taught me to wait a while when another update is 
released.

I've experienced this bug. Provided you have your accessibility shortcut set 
for VoiceOver, recovery is pretty simple. Click the home button three times 
quickly if your device has one. If not, do this with the action button on the 
right side of your iPHONE. You need to do this once to turn VoiceOver off 
completely and then again to turn it back on. This starts it fresh and it 
should work as normal. Sometimes, you need to triple click the home or action 
button a few times before VoiceOver comes back. Just be certain you leave a 
pause between sets of three clicks. Half a second's wait should do it. The 
danger here is that you may click fast enough so that five rapid clicks get 
registered. This activates the emergency SOS feature unless you go into 
settings and disable call using side button. If yOu disable this, you don't 
have to worry. Should you need to use emergency SOS and you've disabled the 
call using side button option, simply hold the side button and a volume button 
down for around a second and a half. A menu will pop up and you can either shut 
down or activate SOS mode. Hope this helps.

Or turn off VoiceOver, and ask Siri to call someone? This way, we could see if 
it’s a VO bug, or a general one. Or also ask the person to hang up first since 
we’ll have VO turned off? ON my SE (I knock wood this won’t happen), but so 
far, no bug as such!

Wow, I can eventually confirm this bug. My phone ran into recovery mode just a 
few moments ago resetting it while plugged in.

This happened to me. Iphone 8+. I called Apple Accesibility because I had not 
read about this problem. They advised me that the recommended solution was a 
hard reset. But they further recommended to do the hard reset with the phone 
unplugged because sometimes the phones are going into recovery mode instead of 
rebooting. He said that this shouldn’t happen unless plugged into a PC, but it 
most definitely was. We did it with mine plugged in, and sure enough it did go 
into recovery which forced me to do the procedure again unplugged. Having done 
it, I’ve had no more difficulties and it has been 24 hours.

I had a client who was on the beta of iOS 13.1 last week who has an iPhone 7, 
who experienced this issue several times.

When he came in for our training session, I called his phone and I answered it 
and the phone worked fine.

I called his phone again and he answered it and it locked up.

He did the power button/home button reboot.

After that, he didn’t experience it again.

It certainly is not a wide spread issue.

But, if it keeps happening, you will need to deal with Apple directly.

 

You could try a reset of network settings I suppose but if it keeps up after 
that, then it is time to contact Apple.

sometimes turning off and on voice over a few times fixes it, other
times I end up having to reset the phone. I can't wait for this one to
be fixed because it happens a few times a day.

It is no the iPhone which crashes totally, this bug seems to effect Voiceover 
and other sounds. I have had this happen, but afterwards my sighted wife said 
everything worked as normally on the phone except there was no sounds, i.e. 
Voiceover obviously was not talking, but also if I said Hey SIRI there was no 
triple tone to indicate SIRI was listening, but on the screen it came up and it 
was listening, when my wife called my phone did not ring, but on the screen she 
saw her incoming call and could answer it etc. Interestingly enough when this 
happens all vibrations are also disabled including the little vibration you get 
when you flick the mute switch and which is usually a good way to see if the 
phone is still on when Voiceover goes away. The only vibration you get is a 
very small bit of haptic feedback when you press the home button, at least for 
me I get it because I have an iPhone 8 with the virtual home button. Not sure 
if you would get anything on an iPhone 10 or, for that matter, on a 6S or 
earlier which still have the mechanical home button.
Toggling Voiceover off and back on occasionally fixes this, but not all the 
time. Sometimes it’s even gone after I call the phone from my landline at work 
or ask somebody to call it and I answer that call and then hang up.

I had a client who was on the beta of iOS 13.1 last week who has an iPhone 7, 
who experienced this issue several times.
When he came in for our training session, I called his phone and I answered it 
and the phone worked fine.
I called his phone again and he answered it and it locked up.
He did the power button/home button reboot.
After that, he didn’t experience it again.
It certainly is not a wide spread issue.
But, if it keeps happening, you will need to deal with Apple directly.

You could try a reset of network settings I suppose but if it keeps up after 
that, then it is time to contact Apple.

I think I experienced this a couple of nights ago. I don't get many
calls, but when my wife called me back after us getting disconnected,
I was not able to use the magic tap gesture to answer the call, and
did not get any feedback from VoiceOver at all. I did not have to
totally restart my phone though, I just restarted VoiceOver and
everything was fine. I'm not sure how one would restart their phone if
there was no speech at all. While this behavior was annoying and
frustrating, I don't get many calls so I can live with it until Apple
fixes the issue.
