People have given you good advice on how to fix the problem.

I had the same thing happen a couple of times and couldn't figure out what was happening, so I pulled the shaft to see what was going on.

I was surprised to find that the shaft was fully seated, but the ferrule had been pushed up. I'm guessing that the curing epoxy did it. The ferrule was a very tight fit on the shaft (before anybody asks, the shaft had been prep'ed underneath the ferrule and a light film of epoxy applied to the shaft before the ferrule was slid on).

I started putting a strip of masking tape down the back of the club, from the hosel to the shaft above the ferrule, to prevent the ferrule from rising. I've never had a problem since.
