I might differ with a few. If the head is aligned with the grip it is almost for sure ferrule creep. I would guess that the ratio of ferrule creep to loose head, based on my experience would be ?? 20:1 ?? Maybe more.

I have seen the separation you are talking about and agree with the other folks, this is more than likely a break down in the epoxy, thus a loose head. The first set of clubs I ever owned taught me this lesson. About three months after the purchase the space between the 4I ferrule and the head began to increase. I thought it was the ferrule loosen until the head separated one day at the range. A good cleaning and reassemble and it has worked fine. A couple of other irons had to be redone also.
