I find the D2 to be one of the easiest of the Titleist bore throughs from which 
to remove the shaft. I've done quite a few of them using Golfsmith's heating 
rod technique...

1. Drill out tip plug
2. Place club in shaft puller
3. Heat about 2" to 3" of heating rod tip to cherry red
4. Insert heating rod in tip
5. Apply heat to hosel and heel area with butane micro torch for about 
30-seconds
6. Attempt to pull shaft
7. If shaft doesn't pull, repeat steps 3 to 6

I've found that shafts originally installed by Titleist usually pull with one 
or two sequences. Reshafts usually take more sequences depending on the type of 
epoxy used.

Hosel looks like it is a separate piece than the head.  Is there anything
