Yes, I believe so. I haven't read the articles about this in detail, but I think someone faked his details on the (written) donation form from the relevant charity. In addition, there has been some error with the confirmation letter he should have received from the charity - either he gets so much junk mail that he just ignored it, or it was never sent. I also have a vague memory of reading something about how there's been a change in the UK so that direct debits can be authorized in some circumstances even if they aren't in writing - but I could be wrong about that.
