Interesting.  I think that works pretty well - my single shared key can
handle a held call on x-line-id 0, and an active call on x-line-id 1 - I
guess that explains why.  I haven't pushed it any further than that as I
don't have enough phones.  Where it breaks down is when two calls are
held - it can't pick which one to unhold.
