I ran into this a long time ago from user perspective. I now tell people, 
"Think of it as a cell phone, press send." I think I also suggest they dial and 
simply pick up the handset (which does the same as pressing dial). I tried 
dialplan manipulation and it's not worth the effort IMO. In my area the number 
of NXX's starting with the number two (2) are overwhelming, and dialing offhook 
with a dialplan that see a match for internal calls without waiting for the 
othr digits is more problem than its worth. I don't think mine has a timeout 
function and dial if its on the reciver. I know I can add th timeout feature in 
sipxconfig for the phone (or phone group).

Not the kind of answer you might have been looking for but to manually edit the 
dialplan on the phone means potentially having to edit it every time a new NXX 
is placed into service locally, which suck-eth. 

The local dialtone the phone plays is not exactly your friend, in that it gives 
the user a false sense of "analogue-ity", when it is digital. It's a behavioral 
change and difficult to get used to. Just keep saying, "think cellphone...".
