Yes I have. I've blown 2. Audio Codes support didn't acknowledge any issues. I 
too was forced to buy a whole new unit as they were out of power supplies. I 
wonder why...

I did the same as you. I bought an expensive power conditioner that sits after 
our already expensive APC UPS that does the same thing. I haven't had an issue 
since, but that isn't saying much. Until the "incidents" it worked fine for 
year, then we lost 2 supplies in under a month. At the time of blow out #1 we 
were having an alarm installed in the data room, but they were not plugged into 
our equipment. At the time of number 2 I was doing some rewiring on the rack 
and it blew. Scared the hell out of me cause the crackling was so loud... :)

The only thing I can think of is the ceiling was opened above the rack so the 
temp rose by maybe one or two degrees. Picking at straws here because I can't 
find anything else.

Please keep me in the loop about what your vendor finds, I was nearly skinned 
alive for our phone system being out for 2 days the last time.
