The outlets test correctly and do include a ground. The racks in which the 
devices are installed are also grounded. The CO stuff, I'm not sure about. Any 
advice on what is considered proper grounding?

We have several Audiocodes M1K gateways deployed for several clients and most 
have been running for over a year without issue.
We've found them to be very reliable and haven't had the issues in which you 
describe.
Are your M1K's and incoming CO lines properly grounded?
