I have VOIP.MS trunks that work with user/password. Interesting that yours
says checked, as that is what it used to say. Mine today, and another last
week said default unchecked.

Register on initialization (Default: unchecked) (cut and paste from the
screen)

Are you sure you have the Voip.ms account registered to the same server
account that you have your ITSP sip trunks registered to?

Under Device, Gateway, your gateway, under ITSP options, click the checkbox
for "register on initialization" box. It is no longer defaulted, and it
should be.

"Register on initialization" IS checked.
On the right side of the screen it says:
"(Default: checked)"

The voip.ms portal has the following option:

To use SIP by IP authentication instead of dynamic register, use the sub
account section.

Should I try this method to authenticate by IP address instead of
user/password?

On my sipXecs 4.2 system I did the following:
Internal SBC Route
     sipxbridge-1 10.8.158.121
     SIP settings all default

Gateway
     SBC Route: sipXbridge-1
     Address: toronto.voip.ms
(email from voip.ms said sip.ca2.voip.ms was deprecated
         replaced by toronto.voip.ms)
     Dial Plan: Long Distance
     ITSP Account
         User Name: xxxxxx
         Password: xxxxxx
All other settings default.
