I'd start with sipx-trace.....wiresharks are great but are a bit large to be 
posting to the list.  I forgot what the current limit is...but its not much.

Most of the time a sipx-trace has plenty to at least see the issue.

Have you checked your packet sizes?  A lot of ITSP do not like TCP and only 
accept USP.  If you packets are getting past 1500 limit they will 
auto-magically become TCP and your ITSP might be ignoring it.  

I've also seen where people set the firewall to only forward UDP ports 
5080/5060 and not TCP ports as well.  In which case onces the packet becomes 
TCP its lost in never never land.
