Not seen it on a production system.

you had the problem of 4.3.0? If so, boot-looping would occur if the config
file was invalid or the wrong files were uploaded (bootrom/firmware) or had
bad checksums (or wrong download type, as in split or not in polycom-ease).

The correct way to see what would be wrong would be to connect the phones to
a working, production version, wipe the phones to defaults from the menu,
and let them get a fresh config.

I think the issue you probably had was an incorrect syntax in the config
file in a development version.
