I use Vonage and have been doing so for the past 3 months.

It's incredible.  Although, you need a pretty fast connection going out
(at least 90kbps) if you want crystal clear quality.  They have a nifty
throttle system (which you control) where you set the amount of outgoing
bandwidth you want to use.

Their customer service has been impeccable thus far and for calls that
fail (rarely) they will credit you without asking too many questions.
