Before I start in on an extensive response, which abuse desks
have you worked for? The reason I ask is that the (largely
unwritten) Best Current Practice varies depending on what
kinds of services they provide. For example, Hotmail's abuse
load is extremely different from UUnet's, and subsequently so
are their policies.
