I think this first appeared in 1996 when the current format obstruction
rules were added.  The paragraph in 1994 did not include the overhead
doors in the list of obstructions (13-1994, 4-4.1.3.2.1) - just a
reference to obstructions > 4 ft wide.  Perhaps others have more insight
on this topic.
