Does your community development plan allow for office in single family homes? If you're not changing from and R to a B classification what is your question. How many new homes don't have home office's? Just because this one adds a waiting room, doesn't really increase the risk or occupancy. I can't see a shrink with 3 or four nurses, a receptionist and 10 or 12 patients backed up. Heck we just did a group home for "Challenged" clients with 13D as required by NFPA 101. You may have one or two 'challenged people there at one time but certainly not 10. So two head 13D is ok in my opinion. Now onto the Codes.
