I am still struggling with similar issues too - IPeng showing no players, and needing to restart squeezebox.exe process (LMS) in Task Manager. I have found it all very difficult to pin down. I can go 3 weeks without issue, then it comes back. I just checked my LMS plugins and image browser was back too....now I have cleared it, and I'll keep an eye on it. Restarted a Controller too.
Could you please send me a copy of your state.prefs file? You can find it in the Prefs' plugin sub-folder (see Settings/Information).
