Current configuration

Crescendo G4/800
Orange USB/FW PCI card
IXMICRO ix3D Ultimate Rez with version 2 
400MB of interleaved FPM RAM
OS 9.1

I have a problem with the above configuration, since installing the G4
card. My monitor comes up green with or without extensions, and looks a
mess. Fonts all jagged, colours all over the place ...

Oddly, when I ZAP the PRAM the problem is gone, but on next normal
restart it is back. Any ideas? 

When I upgraded the operating system, installing OS9.0.2 proceeded as
advised, in that I was able to install the Universal system, but in the
upgrade to 9.1 there was no option for selecting the universal system.
How do I find out if the universal system is installed? And if not can I
find the upgrader which allows me to install the universal system?

Will I need to disable the internal Cache (J38 ?) as per previous
discussions on this list?

Also I am trying to understand how the video extension software works
and what is really needed. 

Installing IXMICRO ix3D Ultimate Rez version 2 sw places the following
in the extensions folder:
ix3D Driver Update
ix3D RAVE Engine
ix3D Video Memory Manager

Are the following extensions needed?

Apple IX3D Graphics Accelerator
Apple IX3D RAVE Engine
Apple IX3D Video Memory Manager

ATI 3D Accelerator
ATI Driver Update
ATI Graphics Accelerator
ATI MPP Manager
ATI Rage 128 3D Accelerator
ATI REsource Manager

Built-In Graphics Accelerator
9600 Graphics Accelerator
