Do a find for Dr. in the salutation field to get a found set of the problem cases. Then do an FMPreplace on the field using a computation that keeps everything but the first four characters. You might want to put Dr. in the prefix field of all of those records. My experience is that for most purposes PhD's don't regularly use the Dr. (me for instance) while medical doctors almost always do.
