I took another stab at making it work out of the box. If you uninstall and reinstall squid and turn on transparent caching, it works fine. Have not had a chance to go through all of the features to ensure they work however, so start small with simply caching. Turn on features one at a time. If you break squid you'll have to turn back off the feature that broke it, etc. If you leave it simple, it works good.
