What do you mean, OS9's disk stuff can't initialize the floppy? If you put the floppy in the drive, I would expect, if it's unformatted, that the Finder would ask you if you want to initialize it. Or, if it's already formatted, then it should show up on the desktop, and you can select it, and then select Erase Disk from the Special menu to reformat it. At least, that's how an Apple built-in floppy drive would work; again, no idea how it might differ for a USB floppy drive, but if it does differ, the USB floppy drive's manual should make that clear.


The program that copies a disk image straight to the floppy is called Disk Copy 6.3.3 and you can download it here, if you don't already have it:
