Some of the 756 minutes are mobile to mobile?  Individual plans don't
include m2m by default, so they would be rated as anytime minutes. 
However, if you are paying for m2m, they should not be included.

I do think loyalty minutes are available each month until they expire
(check mytmobile for the expiration date) as opposed to bonus minutes,
which I think are one large chunk that you can spread over a few
months, but don't reset every month.
