All of the above.... I'm lucky enough to have a few yarn/fiber stores 
on a 30 mile radius from my house.  Plus whenever I travel I try to 
find yarn/fiber stores.  I have travelled as much as 5hrs (Maryland 
Sheep and Wool Festival) due to my interest on sheep and fibers.  I 
have also asked a friend to pick and pay for a bunch of silk yarn 
while she travelled to London.

>Do you use mail order? (And does this mean by Internet or by phone?)

Yes.  Both net (primarily e-bay) and phone orders.

>DO you make what you need(spinning yarns/making cards, etc.)?

Yes, again both as I love to spin as much and sometimes more than 
Twing...  Plus I do indulge on my interest for woodworking and enjoy 
building weaving equipment.

>If you don't have a local yarn shop, have you found any other sort 
>of shop with supplies)?

Walmart, Michaels, and fabric stores carry crochet cotton and 6 
strand embroidery floss as well as silk sewing thread.  Needlepoint 
stores sometimes carry 12 strand silk floss.  I have experimented 
with  all of them as tw warp.

>Do you wait till local/regional conferences and buy in bulk there?

No.  Buying in bulk usually means I'll get sick and tired of the 
string and I never know what I'll want to weave next year, err next 
decade, since it seems I have enough string to last me for about 5 
years...
        But as the saying says.  The one who dies with the most 
string, wins :-)
