We have a couple of single-headed NetApp FAS 3020s.  I've only see one of
them die.  This was after we added a new shelf on a live filer, and the
firmware version didn't play well with the existing shelves, and many (many!)
disks dropped off-line.  A second head wouldn't have helped.

On the other hand, being able to do ONTap upgrades by rolling over between
heads (versus scheduled outage) would be nice.
