My understanding is that below the threshold, equal time applies.  Above the 
threshold, equal rate applies.  So it's a kind of "have your cake & eat it, 
too" approach, rather than being forced to pick one resource allocation 
mechanism vs. the other.

I found the manual on Zendesk (thanks, G. Nicholas), and on page 78 it says 
that Level 1 is MCS 3 (downlink) and 6 (uplink), and Level 2 is 9 (down) and 10 
(up).

It doesn't explicitly say what happens to the weak UEs (below the threshold) 
other than 'protection of the system utilization resources.'

IIRC, Dave said that below the threshold, Equal Time applied.

This implies perhaps that something like Unequal Time applies? It's not clear.

What SINR corresponds to those MCS levels depends, of course, on your channel 
width.
