I would look in the browser console for JavaScript errors. In Chrome, press 
CTRL + SHIFT + I, and select the Console tab. Reload the /newticket page in 
your Trac instance and report any errors back here.

Another thing to try is temporarily disabling all your plugins. There might 
be conflicts. If TracTicketChainedFieldsPlugin works with all plugins 
disabled, enable them one by one until you find the conflicting plugin.
