All stations are digital now, and switching tends to operate remotely from one of about 30 to 40 engineering hubs/routing stations across the country. Such a gaffe would take place at one of those hubs. A computer is supposed to be programed to know which router to select, which vyvx (or other dedicated land line) feed to use, and when to make such a switch. It's basic programing error that causes it.
