I've recently changed jobs, in large part due to Sarbanes-Oxley.  Our
"Universe" systems were different than the big ugly mainframe apps my
company was running everywhere else.

Same here.  We have established a fairly extensive Change Control
Tracking
system with approval tracking at every stage.

Change Request -> Approval
Modification -> Approval
Testing -> Approval
Move to production -> Approval
Final -> Approval
