Take a look at http://dial9.co.uk/ - the aTech guys are fantastic to deal with. Friendly, helpful, knowlegable. Can't ask for much more!
We use the same aTech Telecoms software that powers Dial9 for our internal PBX and it's nice and easy to use whilst still having all the features that you expect. (aTech Telecoms is the on-premise version of the software, Dial9 is the hosted PBX service.)
