I've also had this die/reload/repeat problem several times in the past. In my case it was always because of problems connecting to mySQL but yours may be different. Try looking in the log (/var/log/squeezeboxserver/server.log) and there should be some indication of what's causing it to shutdown & restart.
(PS if the log is long try the command less /var/log/squeezeboxserver/server.log) which allows you to scroll through the log, then press "end" to go to the bottom.
