Mark, I've noticed that there are two different Combo updater files. One will do the upgrade from 10.1 directly, one only updates from 10.1.2 or later. I ran into this issue recently, trying to update my Powerbook (10.1) with the same combo file I had previously downloaded to upgrade my PowerMac running 10.1.3... (MacOSXUpdateCombo.10.1.5.dmg). The one that does the 10.1 upgrade has a slightly different name (MacOSX Combo Update, I think). I eventually succeeded, but this, and the slew of security updates do cloud the issue considerably.
