You could check the asterisks in X-Spam-Level:

if match X-Spam-Level: **********
subject -> [ULTRA SPAM]
else
if match X-Spam-Level: *****
subject -> [SPAM]
else
if match X-Spam-Level: ***
subject -> [POSSIBLE SPAM]
