I had this problem when I upgraded my SE20. At first I thought the
upgrade was fine until I deleted a few e mails and the phone crashed.
I had the same problem, no way could I get the phone to speak at boot
up, eventually I had to get sighted help to input the pass code and
start Voiceover the long way.
I had a look through settings and noticed that the accessibility
shortcut was turned off, which it most certainly wasn't before the upgrade.
I went into the shortcut settings, moved Voiceover up to the top,
probably not necessary but I wasn't taking chances and swiped down to
activate double tapped and exited settings.
I rebooted my phone knowing/hoping that at least the triple click on
the home button should get me running if Voiceover didn't start, but to
my surprise Voiceover came back up as the phone re-started.
I would suggest that anyone who has experienced this problem double
check their settings just in case.

In my case, the forced restart just brought me back to the screen where I would
enter the passcode, and there was still no speech.
I lost speech upon boot up again tonight, but managed to luck into entering my
passcode correctly after being locked out for the 15 minutes. When I called
Apple Accessibility, the person I spoke with, had not heard of the bug, but he
did take my name and file a report. When I mentioned about sending some type of
logs to Apple, he said that was for a specific issue and not one that was
happening to several people, as I had mentioned that some people on various
blindness related email lists also had the problem.

Interesting. Tell him to get screen recognition out of his rotor. Screen
recognition, thankfully, now has different clicks, to show a user they're in
