If the volumes were not formatted with the latest maintenance for ICKDSF,
could this cause the intermittent problems.  VM will have problems with
volumes that are not are not correctly prepared...  If this is the case,
would a DDR from the incorrectly prepared volumes to correctly prepared
volumes resolve the formatting problem?  Or would I have to reinstall
everything on correctly prepared volumes?

I have applied the "Stacked Recommended Service Upgrade 5101" (not sure what
RSU that corresponds with) and I applied the 1750 PSP bucket.  The PSP
bucket did not have any additional maintenance for CP.  It had additional
maintenance for CMS, ICKDSF, EREP, HCD...

You need to get the latest RSU for your zVM 5.1 system
