I don't have z/VM 5.1, just z/VM 4.4.

I think CMS might already be IPLed when you log on to OPERATOR. 

As Mike suggests, when you log on to OPERATOR you might be ipling CMS
but going into CP MODE. Just entering the command TERM MODE VM will let
you enter CMS commands. 

If CMS isn't ipled, you could be entering IPL CMS and then the PROFILE
EXEC try to do stuff with your 191 disk and the "too full" condition is
breaking it. You can prevent the PROFILE EXEC from running. After
entering the IPL CMS command, when the bottom corner says VM READ, do
not hit enter, type in the command ACC (NOPROF  then hit enter. 191 will
be accessed as your A disk but the PROFILE EXEC won't run. If the 191 is
not broken, then you should be able to get to the point where you can
erase your problem child.

But pasting the messages would definitely help.

Can you cut and paste the messages you get when you logon to OPERATOR?
That would really give us something to work with.

By way of history, CMS has long had problems with breaking the file
system on a minidisk when the disk fills up.
It's VERY much better than it used to be, but can still cause problems
on occasion.

I doubt that any of you are the Thick one here. Our Operator is running
in CP. When I try to IPL  CMS to get it into CMS I get 191 full and so I
can't get the Operator into CMS to do an erase. So I am guessing we need
to backup 191 and reformat and restore all but the file I want to erase?
