My favorite came from flyanglersonline.com in their archives (cutt-bait).  It is real simple or real complex however you want to make it.  For simple it is a strip of chartreuse rabbit hair tied on at the head.  I have gone as far as mylar bodies crystal flash with the rabbit strip and epoxied head with 3d green eyes.  All patterns have caught fish.  Not sure if it simulates a squid or a candlefish but in moving water it looks great.
