Mm.  Bell Canada has a service that I use at home, called Internet Call 
Waiting: similar in function, but without the need to call a second number. 
 
Basically, I have a small Java applet that I load whenever I'm online.  If a 
call comes in, the caller's number and name display in the applet, and I 
have three choices: disconnect immediately; click a button that will 
answer the phone and play a canned message saying that I'm on the 
phone but will call right back; or click another button that will activate 
voice mail and let the caller leave a message.  Works OK, and the monthly 
cost is comparable to what you note for Pagoo.

Might be worth asking various telcos down south of the border if they 
offer such a service.
