No, fixed IP is not required. A little more detail on how it works:
When I start my browser I have it set to automatically load the Bell ICD
(Internet Call Display) applet page. This is a small "chrome-free" window
that I leave minimized in the taskbar while I'm online.
If a call comes in, Bell's switching equipment intercepts it to the applet,
showing me the name and number of the caller. At this point I can answer
the call, forward it to another number/my voice mail, or play a canned
message advising the caller I will call him back (or asking him to call me
back.)
If it do take the call, the applet reconnects me to my ISP after I hang up,
and even reloads whatever page I was viewing on the Web at the time,
assuming I was in the first place. It's all quite slick.
As for IP address, the system just uses the dynamically assigned one from
my ISP, as far as I can tell.

Most of the links on that page work OK if you're coming in from outside the
service, except for any ending 'default.htm' (i.e., "Return to ICD Main
Menu", and nav links in left column.) These will bounce you back to a
generic www.bell.ca front page, and toss you out of the ICD section.
The links to the FAQ and the screen captures work, though.
