Do a search and you'll find many polling survey programs.
If you insist on letting someone else control it for you, you'll find
that there too.
The best polling program I've seen is one called Poll-it v2.0
The basic version will do the IP checking to block multiple votes
for you. The professional version has many more options and is
well worth it if you plan on doing a lot of surveys/polls.
