Hello Ken,
You should be getting back at minimum the segment number in the AK302 field. With luck you get the loop number in AK303 and the segment element in error in AK401.
If the payer sends back a 997 (not all do), we archive the transmitted 837 file using the ST02 value as a pointer. The ST02 we sent is returned in the 997’s AK202 field.
When the 997 comes back with errors, we simply read in the archived 837 and match to the original segment/element sent. We report to the end user the claim and line of service that contain the error so they can make corrections and refile.
Once this has been done, or if the 997 comes back without error, the archived 837 file is purged.

Once you have the right format for the 4010A1 file, you then have to decipher the 997 file. When a file is rejected, you get a line number along with a segment name.
If the segment is N3, that could be for any address on any claim. Most people have a temporary claim file that gets transmitted to the carrier. Once received, it gets deleted. I could be remade and then you can get to the line number.
Would it be possible to get them to change the line number to the loop number, i.e. 2310D? How does someone go about this?
