Yes I confirmed that WUG is set to monitor the device and that Enable
Alerts is checked off
Poll Frequency is 1
Poll Timeout is 5
Time Period is 7 days a week 24 hours
Any other suggestions? Just seems odd that the test of the group is
successful, but when applying to a device, it doesn't work

You may want to check to be sure that WUG is set to monitor that
particular device and that alerts are enabled for it.
