This is now perfectly reproducible locally. ( We were caught out by $wgInvalidateCacheOnLocalSettingsChange ) It appear to happen for the reasons we all expected. There is still an "old" version of termbox in the ParserCache which ovbiously contains place holder that ExternallyRenderedEntityViewPlaceholderExpander doesn't know how to expand.
