Hi James,
Why not use the FORM feature in HTML? E-mail is automatic, when you hit
"submit".
You will have to do some treatment on the resulting data at the receiving
end, but you can make the form as nice as you want (Frontpage, Dreamweaver,
even Notepad ;-).

Are you just trying to save in PDF? What about trying Open Office - save
as PDF.
Our graphic artist is creating forms in PDF that will be given to
experienced teachers who will be evaluating student teachers in the
field. They need to be able to save the data and e-mail the completed
forms to a program coordinator. The graphic artist created some beautiful
forms then discovered se couldn't save the data. Some of the teachers will
be using MAC's with OS-X and can open the forms in Preview and save the
data. I'm looking for a similar solution for the Windows users. I'd like
it to be free
